How to prepare a useful product concern report
Describe what you saw
You do not need to prove anything before reporting. Describe the concern accurately and leave out unrelated private information.
- Product name, code, batch and relevant dates.
- Where and when the item was bought.
- What appeared unusual or inconsistent.
- A clear photo of the packaging, where available.
- Any response already received from the seller or manufacturer.
Keep what matters
Keep the product, its packaging and your receipt. They may be needed if a case officer or the responsible business asks for more information.
Follow up privately
You can report without an account. Anonymous reports receive a private follow-up code. Keep it safe, because PIVERS cannot show it again.
Case officers review every report. Your identity is never published and is shared with a business or authority only when necessary and lawful.
If someone is hurt or in danger, contact emergency services first. PIVERS cannot respond to emergencies.
